This form is used when adding new service, or when relocating, repairing, retiring or upgrading existing electric service at a residential or commercial property. It is typically completed by electrical contractors.

If you are instead seeking to simply open a Central Hudson account at a property where an electric service line already exists and does not need to be relocated, repaired, etc. then please complete the Residential Start Service application or Commercial Start Service application instead of this form.
